Set an Appointment

Please set an appointment with a Family Advocate in your area to complete your application. Please bring the following to your appointment. If you do not bring proof of all income, you will be asked to reschedule:

• Your child’s birth certificate
• Proof of annual income (last year’s tax returns, W-2s, or last month’s pay stubs work well)
• Your child’s immunization record

Refer a Friend or Family

When you have finished setting up an appointment to complete your application please consider referring a friend or family member to our program. You can submit the referral here. We will reach out and invite them to apply for the program. The referral can be anonymous. Thank you.
